First & Fourth grade: 11:35am-12:25pm
Third & Fifth grade: 12:05-12:55pm
Student lunches are $2.80 this year, milk is $.55.

TIME TO GET YOUR CHILD’S PHYSICAL EXAM AND IMMUNIZATIONS!
Now is the time to get your child’s school entry requirements completed!
A physical examination is required in the following grades (must be dated within one calendar year of the start date):
• Early learner programs
• Kindergarten
• 6th grade
• 9th grade
• New to Illinois - All Grades
Immunizations are given in accordance to the CDC schedule and IL law.

UNIT 5 STUDENTS WILL NOT BE ALLOWED TO START SCHOOL WITHOUT THE REQUIRED IMMUNIZATIONS
and PHYSICAL EXAM.
Make an appointment NOW for a physical and immunizations! Avoid the rush. No appointment cards will be
accepted in place of a completed physical exam form and immunization record.
